Back to Blog Couple comparing Medicare Advantage and Medicare Supplement plans

One of the most common questions I hear from people approaching Medicare is: "Should I choose Medicare Advantage or Medicare Supplement?" It is a critically important decision, and the answer depends entirely on your personal health needs, budget, and preferences. There is no single right answer for everyone, but there is a right answer for you.

Let me walk you through the key differences so you can make an informed choice, or better yet, bring these questions to a no-cost consultation where I can run the numbers for your specific situation.

What is Medicare Advantage (Part C)?

Medicare Advantage plans are offered by private insurance companies approved by Medicare. When you enroll in a Medicare Advantage plan, you still have Medicare, but your coverage is delivered through the private plan rather than through Original Medicare.

Most Medicare Advantage plans are all-in-one packages that include:

Many Medicare Advantage plans have $0 monthly premiums (you still pay your Part B premium). However, you typically pay copays and coinsurance when you receive care. Most plans use a network of doctors and hospitals, meaning you need to stay in-network for the lowest costs.

What is Medicare Supplement (Medigap)?

Medicare Supplement plans, also called Medigap, work alongside Original Medicare. They help cover the out-of-pocket costs that Original Medicare does not pay, such as deductibles, copays, and coinsurance.

With a Medigap plan, you keep Original Medicare as your primary coverage. The Supplement plan fills in the "gaps." Key features include:

Medigap plans have a monthly premium in addition to your Part B premium. They also do not include prescription drug coverage, so you will need a separate Part D plan.

Key differences at a glance

Cost structure: Medicare Advantage often has lower monthly premiums but higher costs when you use services. Medigap has higher monthly premiums but more predictable total costs.

Doctor access: Medicare Advantage uses networks. Medigap works with any Medicare-accepting provider nationwide.

Extra benefits: Medicare Advantage often includes dental, vision, and hearing. Medigap does not.

Prescription drugs: Usually included in Medicare Advantage. Requires a separate Part D plan with Medigap.

Travel: Original Medicare plus Medigap provides better coverage when traveling, especially outside your home area. Medicare Advantage networks are typically regional.

Who is Medicare Advantage best for?

Medicare Advantage may be a good fit if you are generally healthy, prefer lower monthly premiums, like having dental and vision bundled, and do not mind using a network of providers. It can also work well if you do not travel frequently and want one card for all your coverage.

Who is Medigap best for?

Medicare Supplement may be a better choice if you have ongoing health needs, prefer the freedom to see any doctor, travel regularly, or want the peace of mind that comes with predictable costs. Many people who value simplicity and minimal paperwork appreciate Medigap plans.

There is no wrong choice, only an uninformed one. The goal is to match your coverage to your actual health needs and financial situation.

When you can enroll matters

One critical difference many people overlook: your Medigap Open Enrollment Period begins when you are both 65 or older and enrolled in Part B. During this six-month window, insurance companies cannot deny you coverage or charge you more based on your health. If you miss this window, applying for a Medigap plan later may be more expensive or even unavailable depending on your health.

Medicare Advantage plans, by contrast, can be chosen or changed during the Annual Enrollment Period each fall, or during your Initial Enrollment Period when you first become eligible.

The bottom line

This is one of the most important decisions in your Medicare journey, and it is worth taking the time to get it right. I sit down with my clients, review their medications, doctors, budget, and health history, and then compare the actual costs across both paths. The right choice becomes clear when you see the numbers side by side.
